Responsible for facilitating all patient discharges and transfers from the Inpatient Unit. The Discharge Planner functions as coordinator of discharge plans, collaborating with the patient and caregiver, the patient’s care team, outside facilities and DME vendors, to ensure safe and timely patient discharges and transfers.
Collaborates with health care team members to develop a multidisciplinary discharge plan. Conducts comprehensive discharge needs assessment. Initiates and facilitates referrals for home healthcare, infusion therapy, hospice, other care facilities, medical equipment and supplies. Facilitates transfers to other facilities.
Works collaboratively and maintains active, open communication with physicians, patients, families and other healthcare team members including external health care agencies to facilitate the plan of care in a courteous, professional demeanor. Documents all interventions in the electronic health record in a timely manner. Attends Endorsement and Comfort Rounds.
Assesses and assists in ensuring the safety of patients in guest rooms, outside hotels, and in the home. Arranges family/caregiver and/or sitters to assist in the care of the patient upon discharge to provide a safe environment. Calls patients after discharge to ensure discharge transition was smooth and proactively identifying obstacles that may delay treatment progress.
Conducts Social Work assessment. Addresses needs of both patient and family through counseling, education and community resource linkage serving as an advocate for patients/families in regard to decisions affecting the plan of care.
- Active Georgia Nursing License or Social Worker Licensure
- 2-3 years experience in a clinical setting
- Specialized experience or certifications in related field; oncology preferred
- Superb customer service, telephone, and communication skills
- Ability to show compassion to a diversified group of oncology patients
- Excellent organizational and prioritization skills
- Ability to work independently with minimal supervision
- Must possess the ability to utilize clinical reasoning while ensuring that coordination of services are done in a timely manner
- Demonstrates sound knowledge and actions in the care and decision making for the oncology patient population and seeks guidance appropriately
- Possesses a basic knowledge of personal computer and software for word processing and/or good keyboard skills preferred, or the ability to enter and retrieve data from relevant computer systems
