Job Summary/Purpose:

Responsible for coordination and execution of quality improvement and risk management activities to support organizational goals and strategic initiatives, which include performance improvement, patient safety, infection prevention, medical staff peer review support, accreditation survey readiness and follow-up, data acquisition for external quality reporting (Joint Commission, CMS, etc.), and other responsibilities as required for the effective, safe, and efficient functioning of the department and organization.  Establishes and maintains safety event reporting, follow-up, and use of resulting data for organizational learning.  Investigates and assesses event potential for professional liability, collaborates, and participates with hospital and CTCA Enterprise leadership as needed.  Directs and supervises department stakeholders and workflow. Serves as Patient Safety Officer.

Job Responsibilities:

  • Provides overall direction and structure for the quality/performance improvement function, as part of the WRMC journey to become a high reliability organization.
    • Ensures CTCA Phoenix quality plan development, implementation, annual evaluation, review and revisions that meet or exceed regulatory and accreditation requirements.
    • Co-chairs the quality committee and prepares agendas, in collaboration with physician partner.
    • Leads, or participates, on committees, workgroups, and projects that support quality, safety or compliance.
    • Participates in CTCA Enterprise Medicine and Science calls, data analysis and implementation of enterprise focused programs.
  • Promotes continuous organizational learning and improvement.
  • Leads and facilitates Continuous Process Improvements (CPI) events and instructs cross-functional teams in adapting to and understanding improvement processes, facilitating Kaizen and Rapid Improvement Events, and overseeing process improvement projects.Responsible for translating CPI concepts into practice for identifying process improvement opportunities, and quantifying results and trends.Coaches and mentors others in LEAN principles and methodologies.
  • Collaborates with Medical Staff leadership on Medical Staff quality and performance improvement efforts.
  • Ensures support for peer review and ongoing professional practice evaluation processes
  • Ensures timely and accurate acquisition, management and reporting of chart abstracted data required to meet Joint Commission and CMS requirements.Collaborates with Clinical Informatics on the monitoring of Meaningful Use and any other electronically abstracted quality metrics.
  • Identifies opportunities via data analysis and collaborates with the appropriate stakeholders to initiate needed action to improve results.
  • Manages the Risk Management and Patient Safety functions. Serves as Patient Safety Officer.
    • Leads the Patient Safety Program.Manages the safety event reporting system.Investigates and evaluates event potential for professional liability. Ensures ongoing monitoring for significant events, patterns and trends; and facilitation of debriefings, gap analyses and/or root cause analyses in a timely manner.
    • Collaborates with Patient Relations and others to identify issues that are potential liability risks.
    • Works with other stakeholders to mitigate identified risks. Reports identified issues to senior management and Compliance, as appropriate.
  • Serves as liaison to assigned legal counsel in professional liability actions.
  • Coordinates designated improvement activities to ensure successful accreditation and state surveys and follow up, in cooperation with Compliance. Serves as the official CTCA Phoenix contact for Joint Commission Surveys, applications, and evidence of standards compliance (ESCs).
  • Participates with leadership on policy and procedure development, revision and implementation of those policies.
  • Monitors CTCA Phoenix quality, patient safety and risk activity and results. Ensures that appropriate documentation is maintained and timely reporting to the appropriate committees and Board.
  • Provides data to senior leaders, compliance and other enterprise functions as required.
  • Provides education and training on quality methodologies, patient safety and risk management topics. Facilitates use of clinical quality and safety data for organizational learning and performance improvement
  • Oversees Infection Prevention Program to ensure timely, accurate acquisition, management, and reporting of infection prevention data and activities

Minimum Qualifications:

  • Minimum of 3 years of work experience in acute care quality and risk management is required
  • minimum of 1 to 3 years in a health care supervisory, consulting or leadership position is required
  • Master’s Degree in Nursing, Health Administration or related area is required
  • Current licensure as applicable
  • Strong communication and teaching skills; ability to facilitate improvement efforts, manage projects and implementation plans using a collaborative, team-oriented approach; knowledge of and ability to apply quality/performance improvement philosophy and principles, including Lean Daily Management.
  • Certification Professional in Healthcare Quality or Certified Profession in Hospital Risk Management is preferred
