Job Description:

Under the direction of the Director of Pharmacy, the Pharmacy Clinical Coordinator is responsible for all services, both distributive and clinical, provided by the Pharmacy.  The Clinical Pharmacy Coordinator is responsible for instituting and maintaining clinical and educational programs consistent with the needs of the Pharmacy, Pain Management, Medical, and Nursing and Quality and Risk, departments within the hospital.  He/She serves on all clinical/technical committees as requested by the Director of Pharmacy.

  • Direct development, implementation, and maintenance of clinical pharmacy services.
  • Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
  • Directs the clinical functions performed by pharmacists, technicians and pharmacist interns.
  • Monitors and documents staff performance in area of clinical intervention and outcomes.
  • Provides pharmaceutical/clinical information to health care professional in a timely manner.
  • Provides drug therapy education to health care professionals and patients in a routine and timely manner.
  • Coordinates drug information files, maintenance, and retrieval.
  • Coordinates drug preparation/distribution by maintaining IV and unit dose policies and procedures, quality assurance programs to monitor policy compliance.
  • Coordinates monitoring of individual patient drug therapy by maintaining patient records and screening procedures to identify potential problems such as drug allergies, drug interaction, drug incompatibilities, contraindications, inappropriate dosing and therapeutic duplication.
  • Prepares and presents drug reviews, DUE criteria, antimicrobial stewardship reports, adverse drug reaction reports and pharmacy interventions summary to the appropriate committees.
  • Maintains records on adverse drug reactions and reports this information to the Medical Executive committee and Risk Management and Quality Assurance Departments.
  • Participates in the quality improvement activities of the department.  Collects and analyzes data and maintains documentation of all clinical activities.
  • Coordinates Pharmacokinetic consultation resource to staff physicians for patients.
  • Aids and back fills Staff Pharmacist(s) when necessary.
  • Carries "on-call" pager and responds in a timely manner.
  • Participates in education programs, in-service meetings and workshops as required by the Director of Pharmacy
  • Demonstrates department procedure for the supply and restocking of crash cart and distinguishes age specific needs.
  • Evaluates the medication profile for newly admitted oncology/med/surgical geriatric patients for proper drug dose and schedule to eliminate "poly-pharmacy".

Experience:

  • ACPE- Accredited School of Pharmacy (RPh or PharmD) is required
  • Active and in good standing AZ Board of Pharmacy License is required
  • Minimum of two (2) years of institutional pharmacy experience is preferred
  • Successful completion of hematology/oncology residency is preferred
  • 5-7 years of health system experience preferred with emphasis on clinical pharmacy positions
  • BCOP certification (or eligible) is preferred
