Job Summary/Purpose:

The Clinical Documentation Specialist (CDS) will perform concurrent reviews of inpatient medical records to identify opportunities for documentation improvement.  This position will generate written and verbal physician queries in cases where additional documentation is needed to support the severity of the patient’s condition, specificity of diagnosis and treatment performed, and to ensure that the documentation ensures accurate diagnostic and procedural coding and DRG Assignment.  Responsibilities may also include the education of members of the patient-care team regarding documentation guidelines, including attending physicians, allied health practitioners, nursing, and case management.  The CDS will act as the liaison between the coding professionals and the medical staff.  S/he will use their strong communication and documentation skills to continuously improve the enterprise documentation efforts

                                                                                                                    

Minimum Qualifications:

  • Registered Nurse (RN), Licensed Practical Nurse (LPN), with 5 years of nursing experience, preferably in Med/Surg, Critical Care, Intensive Care or the Emergency Department or a Registered Health Information Administrator/Technician (RHIA),(RHIT), with 3 years of Inpatient Coding experience
  • 1year experience with a clinical documentation improvement program
  • RHIA, RHIT, CCS, or CPC-H preferred
  • AHIMA, ACDIS or equivalent approved ICD-10 trainer preferred; will require certification after 1 year of hire
  • Experience in education and training in clinical documentation preferred
  • Utilization Review background preferred
  • Obtains continuing education to maintain all certifications and licenses
